Section 1 – Background and Introduction

Choose a health or social care organisation that you are familiar with and briefly describe it to put the rest of the assignment into context. Identify a recent significant change process at the organisation that impacted on work and your department / unit specifically. Word count 500 words

Section 2 – LO1 – Understand the factors that drive change in health and social care services. (1.1 and 1.2)

This section is concerned with external and internal drivers of change affecting a health and social care organisation. Identify the key external issues and internal factors that drove the change process. Within the word limit, you should ensure that you have given examples of the key external motives and internal drivers, using the material provided in the unit as well as other literature sources and policy documents. The narrative should reflect the situation in your own organisation and you should be as specific as possible without compromising professional confidentiality. Draft a PEST and SWOT analysis in tabular format to preserve word count and be sure to critically assess the implications arising from your analysis. Word count: 1000 words

Section 3 – LO2 – Be able to evaluate recent changes in health and social care services (2.1, 2.2, 2.3, 2.4)

This section looks at the output of your section 2 analysis, understanding future changes and setting objectives to deal with change. Evaluate the impact of the change on your organisation and staff with particular reference to how services are organised, the effect on front-line staff and the effect on service delivery.  Assess the impact on other stakeholders such as users of services: direct users; families of those who use services. Consider the benefits of the changes for the organisational and other stakeholders. Construct a stakeholder matrix to assess the impact of change, and identify SMART objectives to deal with the future impact of the change. Propose an organisational response to changes and how it might be circulated. Word count: 1000 words

Section 4 – LO3 Understand the principles of change management (3.1, 3.2, 3.3)

This section of the portfolio requires you to explore/research the contemporary change management principles and theory. Using the material and findings in sections one to three above, explain the key principles of change management and how changes in health and social care are planned.  Identify, and critically assess a theory / model that might be used in the change process and justify your choice.  Assess how to monitor recent changes in health and social care services that might affect the organisation.
